Why this matters

AI agents are no longer a future concern. They are running inside enterprise workflows right now — inside SaaS platforms, embedded in vendor products, deployed by individual teams, and stood up by well-intentioned people who never thought of themselves as building infrastructure. The language obscures what's actually happening. They're not features. They're workers. Autonomous entities that read data, apply judgment, take actions, and hand off work — often without a human in sight.

The governance frameworks haven't kept up. Most organizations can't produce a clean inventory of what agents are running, who owns them, or what they can access. They can't connect their AI investment to measurable outcomes because they don't have a complete picture of what's deployed. And when something goes wrong — when a regulator asks, when an acquirer runs due diligence, when an incident surfaces — the gap between the AI narrative and the operational reality becomes immediately expensive.

Human-in-the-loop isn't the problem. Treated right, it's the most important control layer an organization has. AI agents don't get deposed. They don't sit in front of regulators. They don't sign their name to anything. The human in the loop is ultimately the one holding the bag — which means putting humans in that position without the right infrastructure around them isn't responsible oversight. It's exposure.
